Service Description

Your phone consultation is optional and will be with our Medical provider, who will review your intake form questionnaire which includes health history, weight loss goals, and ensure that you are a good candidate for the medication. They will call you only if they have further questions after reviewing your intake form. Once again, you may also choose to book a consultation but this is optional but completing the intake form questionnaire is required.
